Prepare for Your Visit
1. Fill out your History Form
Please download, print and complete the Medical History Questionnaire before you arrive for your exam. Also bring a list of your current medications and all insurance cards. You can even go the extra mile to update the info in your online portal!
2. View the HIPAA Policy and Sign
Please view the HIPAA Policy, then bring the signed Acknowledgement Page with you to your visit.
3. Choose Optomap Retinal Imaging
Optomap imaging allows your doctor to spot signs of high blood pressure, cardiovascular disease, diabetes, and other life and sight threatening conditions. The Optomap image becomes a permanent addition to your record for better continuity of care year-to-year. Our doctors recommend this be performed annually.
4. Request a Record Release
This is optional. Please download, print, and complete the Record Release Form found below if you would like your medical information transferred from your prior eye care clinic to Vision Care Associates.
5. Virtual Waiting Rooms
No more cramped waiting areas! When you arrive at the office, call us. We'll perform the check-in process from the safety of your own vehicle. You may be asked a few questions to screen for COVID and to ask what visual issues you will need addressed during your visit. Then a team member will greet you at the front door.
6. Masks are a Necessity
Our doctors and team have to be within 6 feet of your for some important measurements. The masks are required to help keep us, you, and our at-risk population safe during those activities. Rest assured we also have plexiglass shields and disinfecting processes in place to keep everybody healthy.
